Evidence of Employer Product/Service competency

This is a letter on company letterhead from your employer showing you have been fully trained in the product or service you are providing to the healthcare facility. In the letter, your name must be clearly stated and the letter must signed by an applicable company official and include their title. If you change companies, you will need to submit an updated proof of training from your new company.

For those in clinical positions, please also submit a Skills Checklist or Training Assessment from your current employer.
